Arrhythmias in patients with Brugada-type electrocardiographic findings

Abstract
Brugada syndrome is characterized by marked ST-segment elevation in the rig ht precordial leads (Bru-ECG) and is associated with a high risk for sudden death. However, it is unclear whether the arrhythmogenesis is caused by th e mechanisms responsible for Bru-ECG. The present study investigated the ri sk of arrhythmias in patients with Bru-ECG by retrospectively analyzing 30 patients (28 men; mean age, 51 +/- 14 years) with Bru-ECG. Aborted sudden c ardiac death (ventricular fibrillation or syncope) occurred in 9 patients ( 30%); paroxysmal atrial fibrillation was present in 9 (30%) patients in add ition to malignant ventricular arrhythmias, and some type of arrhythmic eve nt (aborted sudden cardiac death or paroxysmal atrial fibrillation) occurre d in 15 patients (50%). Of all the arrhythmic events, 93% occurred at night or early in the morning, and 92% had pronounced ST-segment elevation. Thes e results suggest that Bru-ECG may be associated not only with an increased risk of ventricular tachyarrhythmias but also with an increased risk of pa roxysmal atrial fibrillation, and that the arrhythmogenesis may be related to the pronounced ST-segment elevation.
